Azelaic acid is a topical treatment for acne and rosacea available under brand names Finacea 15% gel and Skinoren 20% cream.

Azelaic acid can be applied directly to the affected areas of the skin and it can help improve the appearance of acne in two ways. It can prevent the growth of keratin skin cells which can contribute to acne. When these cells mix with oil (sebum) on the skin, they can clog up pores which leads to spots and pimples. Azelaic acid also has some antibacterial properties which can kill the skin bacteria responsible for inflammation. This two-pronged mechanism can help reduce the severity of acne and improve its appearance. 

Azelaic acid’s efficacy has been proven in clinical studies. This medicine significantly reduces the amount of ‘propionibacterium acnes’, the bacteria responsible for some of the symptoms of acne. Azelaic acid has also shown to be effective in reducing the number of papules and pustules on the skin. Azelaic acid gel also offers the extra benefit of penetrating well into all areas of skin, especially the skin damaged by acne. 

Before you apply Azelaic acid, you should wash your skin thoroughly with water and dry it by dabbing the skin with soft disposable towel. 

Azelaic acid should be applied to the affected areas of skin twice a day in the morning and evening. Approximately 2.5cm (0.5g) is enough to cover the entire facial area. 

The duration of Azelaic acid treatment varies between people but it usually takes up to 4 weeks to see an improvement. Azelaic acid is more effective when used continuously for the duration of treatment.

Possible side effects associated with Azelaic acid use include:

·      Very common (1 in 10 people) – application site burning, application site pain, application site pruritus

·      Common (1 in 10 – 1 in 100 people) – rash, feeling of tingling or numbness (paraesthesia), dry skin at the application site

·      Uncommon (1 in 100 – 1 in 1,000 people) - skin reaction due to an external agent (contact dermatitis), abnormal redness of the skin (erythema); scaling, warmth, skin discolouration at the application site
